Request for Certificate of Liability Insurance Note: The certificate of liability insurance provides evidence of liability insurance to individuals or business entities (requestor) that want to verify that you have current, in force, coverage for your business. We are pleased to accept your request for a certificate 24 hours a day, 7 days a week. In order to request this certificate, please click on the following link and completely fill in the blanks so that we may process your request. In some cases, we may need to obtain permission from our insurance company in order to have a certificate issued.
